Don't most big companies only do some small percent of parts inspections per batch received rather than 100% inspection? Having dealt with major parts manufacturing companies I'm going to guess the outfit that supplied the rods to Porsche had an issue with their supplier or their process that they didn't catch. Or on the more nefarious side the supplier knew of the problem, thought it would be ok and/or shipped anyway...
